本研究の目的は、十二指腸腫瘍に生じているVUSも含めた遺伝子異常を網羅的に解析して、その結果から十二指腸がんの発生に関与する遺伝子異常やシグナル伝達経路を明らかにし、治療標的となりうるActionable遺伝子異常を探索することである。初年度である2022年度は、十二指腸腫瘍 (腺腫、がん)を対象として、がん関連遺伝子160種類のターゲットシーケンスを行い、PleSSisionシステム (三菱電機ソフトウェア社)で解析を行った。111例の解析結果が得られ、腺腫とがんに共通する遺伝子異常や、がんだけに認める遺伝子異常を検証した。111例の発生部位別の内訳は、球部/SDA 30例、下行部 57例、IDA/水平部 24例であった。病理組織学的な内訳は、腺がん 45例、腺腫 60例、その他 6例であった。腺がんの大部分は粘膜内がんで、脈管侵襲を有さず、内視鏡的に切除されていた。全症例のゲノムプロファイルでは、APC、KRAS、GNAS、MDM2、CDKN2Aに遺伝子異常を比較的多く認めた。発生部位別に評価したところ、APC (p=0.215)とKRAS (p=0.325)はどの領域においても有意差なく異常を認めたが、球部/SDAでGNAS (p<0.01)、MDM2 (p<0.01)、CDKN2A (p<0.01)の遺伝子異常が有意に多い結果であった。組織型別に評価したが、有意な特徴は見られなかった。組織型別に大きな違いは見られなかったが、球部/SDAに発生する腫瘍は他の部位に発生する腫瘍とは性質が異なる可能性が示唆され、今回の検討結果は、腫瘍発生機序を解明する一助になると思われた。
