当初包括的高度慢性下肢虚血患者に対して血管内治療と脂肪幹細胞移植とのハイブリッド治療を検討していたが、脂肪幹細胞移植は現在患者の新規登録を中止しており国内では施行できない状況になっている。今回我々は脂肪幹細胞移植の代替として骨髄単核球細胞移植による血管内治療の診療体制を構築し「自家骨髄単核球細胞を用いた血管再生治療」が実施できる状況となった。(JRCT 認定番号NA8150008)自家骨髄単核球細胞移植は全身麻酔科に患者自身の腸骨から骨髄液を採取し分離濃縮し患部に筋肉内注射する方法であり、自家骨髄液を使用することから拒絶反応がないため安全である。また骨髄液の採取は骨髄バンクのガイドラインに則り血液内科医に依頼することで手技の合併症リスクを最小化した。実際の診療では、1年間に約25例の包括的高度下肢虚血患者の加療を行った。全例に血管内治療を施行し創処置や補助療法を含め集学的な加療を行った。救肢できない症例、救命できない症例も3例含まれたがいずれも感染や全身状態不良により移植治療に適さない症例であり、現在まで血管内治療と移植治療を併用できた症例はない。引き続き適正症例のリクルートを継続する。
