羊水塞栓症発症リスクの因子の探索として、臓器特異的アナフィラクトイド反応の病理学的検討を行った。羊水塞栓症の病態として、補体活性化を伴うIgE非依存性の肥満細胞脱顆粒(アナフィラクトイド反応)が主体であるが，全身各臓器における差異は検討されておらず各臓器におけるアナフィラクトイド反応の病理組織学的検討を行った。日本産婦人科医会委託事業として継続中の羊水塞栓症血清診断事業と妊産婦死亡症例検討評価委員会に登録された妊産婦死亡症例から臨床的・病理学的に羊水塞栓症と診断され，子宮，肺，心筋，肝，腎，副腎組織検体が得られた13例を対象にした．各臓器標本でトリプターゼ，C5a受容体(C5aR)の免疫組織化学を行った．トリプターゼ染色は陽性細胞1個当たりの平均染色面積(トリプターゼ陽性面積)を，C5aR染色は陽性細胞数を画像解析し，各臓器間でノンパラメトリック検定を行ったところ、トリプターゼ陽性面積は，肺は副腎・肝より有意に大きく、C5aR陽性細胞数は各臓器間で有意差を認めなかった．結果、AFEでは肺，子宮，心筋に特異的に肥満細胞脱顆粒が認められた。
