子宮内膜症性卵巣嚢胞に対する腹腔鏡下卵巣嚢胞摘出術は病巣摘出や疼痛などの症状改善に有用であるが、手術に伴う卵巣予備能低下が近年問題となっており、実際、高度生殖補助医療（ART）を行う際に良好胚を獲得できない症例も散見する。卵巣嚢胞摘出術既往がある子宮内膜症合併不妊女性における採卵時の良好胚獲得率の評価と影響を及ぼす因子を解析した。1回目の採卵で、49例のうち26例（53.1%）が良好胚を獲得できた。1回目の採卵で良好胚を獲得できず2回目の採卵を施行した20例のうち、7例（35.0%）が2回目の採卵で良好胚を獲得できた。2回連続で良好胚を獲得できずに3回目の採卵を施行した11例のうち、3回目の採卵で良好胚を獲得できたのは1例（9.1%）のみであった。累積良好胚獲得率は1回の採卵で53.1%、2回の採卵で69.5%、3回の採卵で72.3%であった。2回連続で良好胚を獲得できなかった症例のリスク因子には採卵時の年齢が高齢であることがあげられた。卵巣嚢胞摘出術既往がある子宮内膜症合併不妊女性において、2回の採卵連続で良好胚を獲得出来ない場合は、それ以降のART成績は極めて不良である。採卵時の年齢が重要な寄与因子であり、より早期の治療介入が必要であると考えられた。子宮内膜症性卵巣嚢胞は疼痛や不妊、破裂・感染・癌化のリスクなどから腹腔鏡下卵巣嚢胞摘出術が必要になることが多いが、近年の晩婚・晩産化による年齢の影響も加わり、不妊治療に苦慮することが多い。本研究課題である「アレルギー性炎症が子宮内膜症性卵巣嚢胞に伴う卵巣予備能低下に及ぼす影響の解明」は、子宮内膜症女性の妊孕性改善に寄与する可能性があり、現在症例を蓄積しているところである。
