自宅と病院の中間施設として位置づけられた介護老人保健施設では、退所者の3人に1人が自宅に戻ると報告されている。ケアの提供体制が変わる在宅移行期は、身体機能の低下などリスクが高まり、注意が必要な時期である。日本では、介護保険制度創設時から移行期ケアの質を確保するため、退所時情報提供加算、連携加算、訪問指導加算などを算定できる仕組みを構築した。しかし、介護施設における在宅移行期ケアの効果を示すエビデンスは皆無に近い。本研究の目的は、介護老人保健施設における在宅移行期ケアの利用実態を把握し、高い在宅移行期ケアの質に関連する施設特徴を明らかにすることである。本研究で用いるデータは、自治体の医療介護レセプトと施設特徴を含まれる介護サービス情報公表システムのデータである。今年度は、介護サービス情報公表システムに公表されている老人保健施設の加算情報や人員配置の情報の収集を完了した。また、本研究で応用できるケアの質に関連する地域特徴を検証した論文を執筆し、BMC Public Healthに出版した。
