本研究は、悉皆性が期待できる全国がん登録データに基づき、本邦の小児がん患者が受療のため病院へ移動した際の負担を、時間、距離、費用の観点で多角的に分析して報告するものである。世界的に小児がん治療の集約化が進められ、移動負担が着目される中、道路交通網や公共交通機関網などの実際の移動ルートを想定した移動を推定して結果を分析することで、小児がん治療の集約化に資する精度の高い基礎資料の提供を目指す。初年度である本年度は全国がん登録データに基づき、２０１６年または２０１７年に新たにがんと診断された１８歳までの小児がん患者を対象に、自動車と公共交通機関をそれぞれ利用した場合の病院への移動時間、距離、及び、費用を推定した。性別、就学区分、がんの種類、本邦を７地域に区分した地域ブロックなどの層別因子毎に結果を要約した上で、多変量解析で移動時間が増加するリスク因子を検討した。結果、本邦における小児がん患者の移動実態は中央値でそれぞれ、自動車の場合は0.6時間、16.5 km、0円、公共交通機関の場合は1.1時間、18.7km、630円であった等の知見を得た。また予備的検討のためシミュレーションとして、平成２７年度国勢調査町丁・字等別境界データに基づき、各町丁・字より小児がんの拠点病院と連携病院に指定された各病院のうち最も近い施設までの直線距離を地域ブロック毎に小児人口で重みを付けて求めた加重中央値で比較した。結果、地域ブロック間の中央値の範囲が拠点病院では17.2～105.5 kmに対し連携病院では5.9～25.8 kmと狭く、地域間格差の縮小が示された。並行して、全国がん登録データについて追加データを取得する変更申請を行った。結果、診断年が２０１９年までのデータについて、これまで研究対象としていた小児がんと、比較対照として新たに設定した青少年および若年成人（AYA）のがんの情報を取得した。
