本研究では，全国規模のレセプトベースのデータベースを基に本邦における全国的な大腸内視鏡検査状況を評価し、大腸内視鏡検査後に発見される大腸癌(PCCRC:post-colonoscopy colorectal cancer)の現状およびその発生に関わる要因を明らかにすることを目的としている。また、内視鏡PCCRCと内視鏡検査との関連を評価するためには大量の症例データが必要である。そこで、本研究では大規模な症例数を含むレセプトデータを用いて内視鏡検査およびPCCRC発生の全国的な現状を把握し、評価することで新たなエビデンスを得ることを主眼とした。一方、大腸内視鏡検査や大腸内視鏡治療の頻度により検査後の大腸癌発生に影響がでてくるため、本研究では症例毎に大腸内視鏡検査・治療歴を経時的に追跡することが重要である。そのため、異なる病院での内視鏡検査・治療も追跡可能なデータベースを用いること必要であり、本研究では株式会社JMDCの提供する保険者ベースのデータを用いることとした。研究初年度に、レセプトデータを利用するためにJMDCとのデータ使用契約を締結した。また、本研究に関する倫理審査についても所属する施設で申請を行い、研究実施に向けての準備を整えた。今後は研究実施計画に基づき必要なデータを抽出・受領し、取得したデータをもとにデータクリーニングおよびデータセットを作成し、解析を行っていく方針である。

This study aims to evaluate the status of colorectal endoscopy in China and to clarify the main causes of post-colonoscopy colorectal cancer (PCCRC). A large number of cases are necessary for evaluation of the relationship between PCCRC and endoscopic examination The number of large-scale cases in this study includes the status quo of endoscopic examination and PCCRC development in China. On the one hand, the frequency of colorectal endoscopic examinations and colorectal endoscopic treatments has an impact on the development of colorectal cancer after examination. In this study, it is important to track the history of each colorectal endoscopic examination and treatment over time. This study was conducted by JMDC, Inc., a provider of endoscopic examination and treatment in different hospitals. At the beginning of the study, JMDC signed a contract. The ethical review related to this study involves the preparation of the application for the design and implementation of the study. In the future, we will study the implementation of the plan, and we will take the necessary measures to ensure that the policy is implemented.
