项目摘要

本研究は外来でがん化学療法に携わる看護師の身体に埋め込まれた実践知を記述することにより解明することを目的としている。そこで、外来化学療法室に勤務する看護師の経験を記述的に明らかにする研究から開始した。その結果、看護師にとって外来化学療法室は「じっくり関われる場」「きちんと話をする場」として経験されており、その経験は長期に個々の患者に関わるという外来化学療法室固有の状況に支えられていた。さらに看護師の経験には「頑張っている患者の力になりたい」「がんを生きる患者の生活をどう支えていくのか」というそれぞれの看護師の看護の拠り所となるものが埋め込まれていた。加えて、看護師たちは配置転換や患者との関わりを節目としながら「一皮むけた経験」を積み重ねて意味づけ、看護師としての自身の存在価値に気づき、キャリアの課題を乗り越えて発達させていた。また、看護師の経験の研究の中で、看護師には言語化の難しい瞬間的かつ即興的な行為や実践が存在することが明らかとなった。そこで、看護師の直観に関する文献検討を行った。その結果、看護師の直観に関する文献は主に、１）看護師の意思決定プロセスにおける直観、２）患者や家族との関りのための手段としての直観、３）看護師の臨床判断における直観、４）直観能力や直観の経験に整理された。看護師の即興的な行為や実践における直観は、行為的直観によって説明され、こうした哲学的な見方は看護師の直観を理解する手がかりとなりうることが示唆された。
