项目摘要

本年度は3つの研究を行った。第一の研究は、部分条件付き平均モデルに対する逆確率重み付け推定量と二重ロバスト推定量を、（1）健康状態の単調欠測がMARで，QOL値の非単調欠測がMARの場合、（2）健康状態の単調欠測がMARで、QOL値の非単調欠測がNMARの場合に、セミパラメトリック理論にもとづき導出した。得られた推定量は、それぞれの仮定のもとで、漸近正規性を持つ一致性であり、漸近分布の漸近分散も導出した。第2の研究は、セミパラメトリックモデルのもとで回帰モデルのデータへの当てはまり度合を評価する適合度検定をいくつか開発した。開発した適合度検定の多くは、追加共変量の回帰パラメータに対する一般化スコア検定の形を持もつ。一般化スコア検定は、スコア検定をスコア方程式だけではなく推定方程式にもとづく検定に拡張したものである。シミュレーション実験を行い、第一種の過誤確率の名目水準への維持と検出力の観点から、開発した正規化残差平方和検定が他の手法と比較して優れていることを明らかにした。第3の研究は、実際の医療経済評価において、QOL値の分析を行った。HER2陽性高齢乳癌患者に対する術後補助化学療法の医療経済評価を臨床試験データをもとに行い、トラスツズマブ+化学療法併用療法がトラスツズマブ単独療法に比べて、費用対効果が良い可能性が高いことを示した。しかし、QOL値の不確実性が結果に与える影響は大きく、QOL値の不確実性を減じることを目指す本研究課題の重要性が浮き彫りになった。
