项目摘要

本研究の目的は、AIによる機械学習を活用した「テーラーメイド型院内急変予測モデル」の開発である。本モデルは、入院患者の生体情報や診療情報に基づいて、個々の患者に対する急変リスクを精密に予測するものである。2022年度には、この目標に向けて、生体情報や血液検査結果に基づいた急変予測アルゴリズムの作成に取り組んだ。「院内急変の発生」や「院内心停止の発生」をアウトカムデータとして設定し、その徴候を捉えるアルゴリズムを構築することが目的であった。この結果、Rapid Response System（RRS）における過去の事例と比較したところ、作成したアルゴリズムのドラフトでは急変予測の一定の精度の達成が見込まれた。現在、「NEXT Stage ER」の本研究向けにマイナーアップデートが進行中であり、カルテデータとデータウェアハウス（DWH）との連携強化に取り組み、さらなる精度向上に努める予定である。この成果は、医療の質と安全性の向上に寄与するという観点から重要性を持つ。院内急変は患者の死亡率を高める一方で、その早期発見と対応は重大な結果を防ぐ可能性がある。したがって、急変予測モデルの開発は、患者の安全確保という観点から極めて重要である。しかし、現段階のモデルはまだ初期段階にあり、患者の個々の背景や診療情報を考慮に入れていない。今後の研究では、これらの情報を組み合わせた急変予測モデルの作成と検証を進めていく予定である。そのための基盤となるデータベースとの連携構築については時間を要しているが、これが実現すれば、急変予測の精度はさらに向上するだろう。以上の点を踏まえると、2022年度の研究成果は、急変予測の精度向上という目標に向けての一歩であり、その意義と重要性は極めて高いと言える。ただし、さらなる精度向上を達成するためには、今後の研究が重要である。
