1.対象;産婦人科で経過観察中の慢性特発性血小板減少性紫斑病の妊婦ならびに出生した新生児。母親の承諾の上で母体血,臍帯血、新生児の血液を採取し,血清を保存した。また、新生児の血小板数を経時的に測定した。2.抗血小板抗体の検討PAIgG,PBIgGはBorneらのPlatelet suspension immunofluorescence testを用い、血清中の特異抗体の検出はAntigen capture ELISA変法ならびにavidin-biotin-peroxidase complex を使用したwestern blottingにより血小板膜糖蛋白(GP)IIb/IIIaに結合しうる血清中の抗GPIIb/IIIa抗体を検出した。いずれも既に測定法は確立されている。しかし、血清中の抗体検出においてはcontrol群においての非特異的反応が強く、そのbackgroundを考慮に入れた際に、患者検体での弱陽性所見が本当の移行抗体によるものなのか判断に迷う状況である。また、母が慢性特発性血小板減少性紫斑病の場合に、新生児で血小板が減少する確率も2-3割と低く、十分な検体が揃わない状況である。今後は各種方法でのデータの特異性を高め、より多くの検体で検討していきたい。
