IgA腎症の発症におけるT細胞レセプター(TCR)Vβ陽性細胞の関与を明らかにするために,IgA腎症およびIgA関連腎炎(紫斑病性腎炎,MRSA感染後腎炎)におけるリンパ球サブセット,とくにTCRVβ陽性細胞の選択性を末梢血および腎組織において検討した.昨年度の検討の中で,FACScanを用いた3カラー解析による末梢血リンパ球のTCRVβ陽性細胞の測定では,IgA腎症群は正常対照群と比較してTCRVβ5.3およびTCRVβ-8の使用が有意に高率であることがわかった.今年度は,腎生検により得られた腎組織を用い,腎間質組織浸潤細胞において,とくにその表面抗原に関する検討を行った.IgA腎症症例38例の光学顕微鏡による解析では,メサンギウム細胞増殖の程度が軽度13例(34%),中等度17例(45%)であった.腎間質浸潤細胞は,27例(74%)が軽度で,6例(16%)が中等度であった.間質浸潤細胞の多くは免疫組織染色によりCD3陽性細胞であることがわかった.さらに,腎間質浸潤細胞のTCRVβ抗体による免疫組織染色では,末梢血リンパ球のTCRVβ5.3陽性細胞数と,腎間質浸潤細胞のTCRVβ5.3陽性細胞数の間に相関があり,さらに3年間の観察時点で血清クレアチニンが1.5倍以上になった症例(腎機能進行群)ではそうでない群(腎機能安定群)と比較して,腎間質浸潤細胞のTCRVβ5.3陽性細胞数およびTCRVβ8陽性細胞数が多い傾向にあることがわかった.以上より,特定のTCRVβ陽性細胞の血中および腎組織中の増加が,IgA腎症の発症および進展に関与していることが示唆された.

The relationship between IgA kidney disease and IgA associated nephritis (purple spot nephritis,MRSA infection nephritis) in the development of IgA kidney disease and T cell receptor (TCR)V β positive cells in peripheral blood and kidney tissue. In the past year's review,FACScan has been used to analyze TCRVβ-positive cells in peripheral blood and spheres. IgA kidney disease group has been compared with normal control group. The use of TCRVβ5.3 and TCRVβ-8 is intentionally high. In this year's study, 38 cases of IgA renal disease were analyzed by optical microscopy, and 13 cases (34%) and 17 cases (45%) were found to have moderate degree of cell proliferation. Renal interstitial infiltrative cells were found in 27 cases (74%) and 6 cases (16%). Interstitial infiltration of cells and CD3 positive cells by immunostaining. In addition, the number of TCRV β 5.3 positive cells in peripheral blood was correlated with the number of TCRV β 5.3 positive cells in renal interstitial infiltrating cells by immunohistochemical staining, and the number of TCRVβ5.3 positive cells in renal interstitial infiltrating cells was more than 1.5 times that in serum at the time of observation during the three years. The number of TCRVβ5.3 positive cells and TCRVβ8 positive cells in renal interstitial infiltrating cells tended to increase. These findings indicate that specific TCRVβ-positive cells are increasing in the blood and kidney tissues, and that IgA is associated with the development and progression of renal disease.
