水痘帯状庖疹ウイルス(VZV)は急性網膜壊死(ARN)と帯状庖疹という臨床像の異なる疾患の原因ウイルスである。ウイルスの病原性の違いをウイルス遺伝子内に検索するため、今回我々はARNの患者由来VZVと帯状庖疹の患者由来VZVの遺伝子多型性を比較した。対象と方法:対象は1991年10月より1998年1月までの間に、横浜市大眼科を受診し急性網膜壊死と診断され、前房水または硝子体液よりVZV遺伝子が検出された5例、および同皮膚科を1996年6月より同年12月までの間に受診し帯状庖疹と診断された4例である。ARNの患者前房水または硝子体液、帯状庖疹の患者庖疹擦過物よりDNAを抽出後、PCR法でVZV PstI変異領域を含む642bpの遺伝子を増幅した。増幅産物はPstIで切断後、切断部位の欠失を検出した。結果:PstI切断部位を欠失したウイルスの検出率はARNの患者由来VZVで1/5(20%)、帯状庖疹の患者由来VZVで0/4(0%)であった。結論:ARN患者由来VZVと帯状庖疹患者由来VZVでウイルス遺伝子内のPstI変異領域に違いは検出されなかった。今後、ARNの発症原因とされるVZVと帯状庖疹の原因であるVZVとの比較には、部位を変えたさらなる遺伝子解析とあわせて宿主側の発症原因解析の必要性もあると考えられる。
