【目的】患者や家族から、暴言暴力を含めたAggression (攻撃)をうけ、ストレスを感じる病院職員は少なくない。先行研究では、早急な心のケアが必要であるとされている。そこで、近くにいる同僚等(ピア)による対応が重要であると考え、ピアの声掛け等の初動対応の意義とその内容を検討した。【方法】平成27年9月に、国立大学附属45病院にアンケートを各10部配布し、実態を調査した。アンケートでは、攻撃を「直接的攻撃」「間接的攻撃」「暴言」「セクハラ」「非表出な攻撃」の5分類とし、攻撃経験が現在の業務に役立っていると思う程度(有意義感)を10段階評価してもらった。一方、ピアからの声掛けの有無とその内容等を回答してもらった。なお、本研究は本院の臨床研究倫理審査委員会の承認を得た。【結果】アンケート回収数は351 (回収率78%)で、有効回答338を分析対象とした。過去1年間で攻撃を受けた人数は全体の70%で、そのうち、ピアからの声掛けがあった割合は66%であった。有意義感の平均値を、声掛けの有無で比較したところ、「間接的攻撃」(n=145)では5.4±2.6(声掛無)、6.1±2.6(声掛有)、「暴言」(n=204)では5.4±2.3 (声掛無)、5.9±2.7(声掛有)となり、声掛けによって有意義感が有意に増加した(p<0.1)。更に、ピアからの声掛けによって高いポジティブ感情を示した群では、心配・共感・承認・状況確認・今後の対応等の声掛けがなされていた。【考察】患者等から「間接的攻撃」および「暴言」を受けた職員は、ピアからの適切な声掛けが心をケアし、その経験を次に生かそうと前向きな教育になる可能性が示唆された。更に、声掛けの内容は本人に共感し、対応を認め、発生した状況を客観的に把握し、今後の対応を共に考えること等が効果的であった。【この研究の意義】ピアの声掛けの重要性を教育し、声掛けを実践していくことで、患者等からの攻撃といった大きなストレスから病院職員を守れる可能性を導いた。
