近年、医療従事者や患者家族への抗がん剤曝露とその影響について注目されているが、患者自宅での5-FU持続投与に使用されている携帯型ディスポーザブル注入ポンプ(以下、携帯型ポンプと略)の取扱いの実態については報告がない。本研究では、薬剤師が調製した直後の携帯型ポンプと着用したゴム手袋、または患者が使用した後の携帯型ポンプと取り外し時に着用したゴム手袋を回収し、LC-MS/MSにて5-FUの定量を行い、抗がん剤曝露の有無について測定を行った。さらに、携帯型ポンプの自宅での取扱い状況と、薬剤師が調製する際の曝露対策についてアンケート調査を行った。薬剤師が調製に使用したゴム手袋の50.0%に5-FUが検出され、平均値は52.3μgであった。また、調製直後の携帯型ポンプでは、ルアーキャップ周囲の21.4%(平均0.131μg)、充填口周囲の28.6%(平均0.731μg)、ポンプ側面の21.4%(平均2.07μg)に5-FUが検出され、これらの箇所は患者または看護師が触れる可能性も高いと推測される。また、調製者対象アンケートにて88.9%が調製時にゴム手袋を1時間以上交換していないことが分かった。患者が使用した後の携帯型ポンプにおいて、使用後ルアーキャップを閉鎖した群と閉鎖していない群それぞれの平均5-FU検出量は、ルアーキャップ周囲では0.044μgと216μg、ポンプ側面では0.069μgと535μg、ゴム手袋では0μgと58.1μgであり、ルアーキャップを閉鎖後廃棄した群の曝露量が有意に少なかった。以上の結果から、調製時にゴム手袋に抗がん剤が付着し汚染が広がる可能性は高く、ゴム手袋の短時間での交換が必須であり、調製後の携帯型ポンプを清拭する必要性も考慮すべきである。また、患者・患者家族が携帯型ポンプの取り外しを行う際、ゴム手袋の着用は必須であり、廃棄時にルアーキャップを閉鎖する必要があると考える。本研究結果をもとに、携帯型ポンプの院内取扱いマニュアルとタブレット端末による患者・患者家族向け教育ツールを作成中であり、これらは多くの施設で活用できるものと考える。
