【研究目的】本研究は, 昨年度開発した, 「小児期高次脳機能障害チェックリスト(以下, チェックリスト)」(澤田他, 2015)の有用性について次の2つの視点(支援者の精神的健康度との関連および支援機関における有用性)から検討し, 最終的にチェックリストに対応した支援マニュアルを作成することを目的とした。【方法】対象 : 6歳～18歳までの高次脳機能障害を持つ子どもの家族(17家族), およびその支援者(担当教諭2名, 医療従事者10名)調査材料 : 小児期高次脳機能障害チェックリスト, フェイスシート(対象児の年齢, 疾患名, 発症時期, その他の困りごと), POMS, Zarit介護負担尺度日本版の短縮版(J-ZBI8), チェックリストの有用性に関する質問項目(学業・生活指導・友人関係に関する配慮, 障害理解, 進路指導, その他自由記述)【結果と考察】チェックリストと支援者の精神的健康度を検討するために, チェックリストの各下位項目の合計得点とPOMSおよびJ-ZBI8の関連を調べるために, 相関分析を行った。その結果, J-ZBIとチェックリストの「行動面」との間に有意な正の相関が認められた(γ=.86, p<.01)。このことから, 子どもの高次脳機能障害の各側面の中で, 特に本チェックリストにおける行動面の特徴が多いほど家族が介護負担感を強く感じていると考えられる。次にチェックリストが支援機関においてどのような点で有用性を感じているかを検討するために, チェックリストの有用性に関する質問について項目ごとの得点を比較した。得点の高い順に並べると, 障害理解, 生活指導面への配慮, 友人関係に関する配慮, 学業面への配慮, 進路指導であった。また, 自由記述からは, チェックリストでは表すことができない個別の対応, 家族への支援方法についてのニーズがあることが明らかとなった。これらの結果をもとに, 特に子どもの行動面への対応についての内容を多くした支援マニュアルを作成した。しかし, 支援マニュアルだけでなく, 患者を取り巻く各支援者が実際に情報共有を行い, 個別の対応方法などを話し合う必要があると考えられた。
