【研究目的】本研究では、静岡県西部地区の大規模医療機関にて運用されている周術期クリニカルパス内の手術部位感染予防抗菌薬について調査し、Evidence-based medicine(EBM)の観点から、不適切な抗菌薬処方の有無を明らかにすることを目的とした。また、クリニカルパス内の不適切な抗菌薬処方を変更することにより、抗菌薬の適正使用推進および使用量削減の実現を目指した。【研究方法】①静岡県西部地区の8施設(浜松医科大学医学部附属病院(当院)、浜松労災病院、JA静岡厚生連遠州病院、聖隷浜松病院、総合病院聖隷三方原病院、中東遠総合医療センター、磐田市立総合病院、菊川市立総合病院)で運用されている手術関連クリニカルパスを収集し、抗菌薬処方の有無について調査した。②各クリニカルパスの抗菌薬処方について、エビデンスの有無を文献検索や書籍等から評価し、「継続」「内容変更(使用日数・抗菌薬の種類等)」「削除」を判断した。③当院のクリニカルパスに含まれる「削除」と判断された経口抗菌薬について、各診療科の医師と協議し、同意が得られた経口抗菌薬を削除した。④③で削除した経口抗菌薬について、当院における四半期毎の使用量推移を調査し、Antimicrobial usedensity(AUD)を算出した。【研究成果】対象の8施設において、眼科の角膜移植術、斜視手術、腎臓内科の腎生検、産婦人科の広汎子宮全摘術、耳鼻科領域の手術などのクリニカルパスでセフカペンピボキシル、セフジトレン、セフジニルが処方されているのが確認された。これらすべての経口抗菌薬が「削除」と判断された。このうち、耳鼻科領域の手術以外のクリニカルパスにおいて経口抗菌薬を削除することができた。セフカペンピボキシル、セフジトレン、セフジニルのAUDは3.12から1.12と、クリニカルパスの変更前後で減少した。

[Objective] This study aims to investigate the use of antibiotics for the prevention of surgical site infection in large scale medical institutions in Shizuoka Prefecture during the operation period, to identify the point of Evidence-based medicine(EBM), and to identify the presence or absence of inappropriate antibiotic prescriptions. The aim is to improve the formulation of inappropriate antimicrobial agents in the pharmaceutical industry, promote the proper use of antimicrobial agents, and reduce the amount of use. [Methods] ① A survey was conducted at eight facilities in Shizuoka Prefecture (Hamamatsu Medical University Hospital, Hamamatsu Hospital, JA Shizuoka Housei Renenshu Hospital, Seiri Hamamatsu Hospital, Seiri Sankouhara Hospital, Nakanishi Hospital, Iwata Municipal Hospital, and Kikugawa Municipal Hospital) using the surgical correlation index and the presence or absence of antibacterial drug prescriptions. (2) For each antibiotic prescription, the presence or absence of the antibiotic prescription, literature search, books, etc., the evaluation, the "content change"(the number of days of use, the type of antibiotic, etc.), and the "removal" are judged. 3. When the hospital's medical records include "removal" and judgment, the doctors of each department agree to remove the oral antibacterial agent. (4) To calculate the Antimicrobial use density (AUD) after removing the antimicrobial agent from the mouth and investigating the usage change of the hospital in each of the four half-periods. [Results] The results of this study were confirmed in 8 settings: corneal transplantation in ophthalmology, strabismus surgery, nephrology in nephrology, total hysterectomy in obstetrics, and surgery in otolaryngology. This is the judgment of "removal" of the antibacterial agent. In addition to surgery in the field of otology and otorhinology, it is necessary to remove antibacterial agents from the mouth. The AUD of the first quarter of 2013 decreased from 3.12 to 1.12, and the AUD of the second quarter of 2013 decreased from 3.12 to 1.12.
