「研究目的」切除不能肝細胞癌(肝癌)の治療薬であるソラフェニブは治療期間が長いほど生存期間が延長することが報告されている。しかしながら、ソラフェニブ治療は副作用が高頻度に発生し、約半数が副作用により治療継続が困難となってしまう。肝機能異常はソラフェニブの投与中止に至る主な副作用の1つとして知られており、肝機能異常を予め予見し、対策を行うことが出来れば、治療期間および生存期間が延長する可能性がある。これまでに、いくつかの有害事象と関連がある指標として遺伝子多型に関する報告があるが、それら遺伝子多型と肝機能異常との関連は検討されていない。本研究では、肝機能異常を予見するマーカーとして遺伝子多型に着目し検討を行った。「研究方法」当院においてソラフェニブ投与開始となった肝癌患者から予め抽出し凍結保存しておいたDNAをサンプルとして用いた。対象遺伝子はソラフェニブの薬物動態に関与の報告があるABCB1(rs2032582), ABCG2(rs2231137)とし、DNAのシークエンス解析を行い、SNPのアレルを判定した。SNPのアレル分布と肝障害の重症度の関連についてカイ二乗検定により検討した。本研究は岡山大学倫理審査委員会の承認を得て行った。「研究成果」当院においてソラフェニブ投与を開始した131例を対象に、各遺伝子におけるSNPとグレード3以上の肝機能異常の関連を調査したところ、ABCG2(rs2231137)でAアレル保持患者は非保持患者に比ベグレード3以上の肝機能異常を起こしにくい傾向を認めた(オッズ比0.317(0.080-1.264))。今回の結果より、遺伝子多型を確認することで肝機能異常が起こりやすい患者を事前に評価できる可能性があり、今後も検討を重ねていく。
