平成19年度の最初に保護者と話し合い,生活地図を作ると共に,どのような機関とどんな内容で連携をしていくと良いかを考え共通理解して,「連携の記録」に一覧にまとめるたことは,必要な連携を整理し,それに沿って計画的に連携ができとても良かった。ICFの図にまとめたことも、将来を見据えた上で今何をすべきかを明確にすることができた。また,高等部卒業までに,A子とA子の支援者双方が,いろいろな活動でまた余暇で外出を楽しめるようになってほしいという「将来の姿」を関係機関と共有することで,外出時でのパニックに対しての支援を一貫して行えることにつながった。さらに,日々学校と家庭でやり取りしている生活ノートの形式を変え,関係機関の方が書き込む欄を設け,家庭-社会資源-学校が本人の状態を共有したり,支援の仕方やそれに対する評価などそれぞれに必要な情報をやり取りしたりすることができ,話し合いを持つ際にも有効であった。こうした連携を通じて,家庭-関係機関-学校が,A子本人の姿を実像を伴って共有できたときに話し合いが深まることが実感できた。その結果,A子の行動にも落ち着きが出てきて,校内で他の生徒と,また社会資源で利用者と同じように外出の活動ができるようになった。さらに保護者が移動支援のサービスを受けることに踏み切るなど,A子の社会参加が確実に広がった。ただ,学校からの情報量は多くなりがちで,そのために必要なことが伝わりきらないこともある。さらに,今回のように長いスパンで関わる機関ばかりではない。したがって,それぞれの機関のねらいをふまえた上で必要なことを伝え合うこと,分かりやすく誤解のないように情報を共有するために「何をどう伝えるか」を常に考えていくことの必要性を感じている。これからも,A子が参加する社会の中で,本人の姿を共有できる支援者を広げていくことで,社会参加につながる連携をしていきたい。
