项目摘要

昨年、私は解剖学実習中に収集した破格例のうち、文献的にも大変珍しい巨大坐骨動脈の存在例を学会報告し、論文として纏めた。本例は下肢の血管外科・整形外科領域の治療法等に有効な報告であると考えられる。本年度はこの研究をさらに発展させ次のようなことを行った。(1)過去2年間に解剖学実習中に見つかった破格例の写真を撮った。まだデータベースとして公開するには至って無いが、それをファイルソフトを使ってご遺体の年齢・性別等の基本情報とともに分類・整理を進めている。(2)今までに見つけた破格例に基づいて解剖学実習用遺体を詳細に観察したところ、本年もまた坐骨動脈の破格を発見した。本例はこれまでなら見過ごしてしまいそうな血管であったが、これ迄データベースを充実させていたため注意深く観察した結果見つけることが出来た例である。これについては今後詳細な検討を加え、場合によっては学会発表も行う予定である。(3)平成19年度の実習中に見つかった先天性腎発育不全の例について共著者として論文を投稿し、受理されたその論文に於いては肉眼解剖的な検討を加えるだけでなく、本学人体病理学講座の協力を得て病理学的考察も加えている。その結果、病理学的にも学術的価値の高い症例であるという評価を得ている。さらにこの破格を伴う遺体を含めた複数の遺体より肝臓組織片を採取し、RT-PCRを行い遺伝子解析を試みた。その結果、破格と遺伝子解析の結果との間には明瞭な関連性はみられなかったが、これについてはホルマリン固定標本のPCRに関して、その条件設定も含めた更なる検討が必要である。今後は更なる破格例の収集を行うだけではなく、過去の例を実習前の学生に対して提示し、人体の多様性に関する認識を深めさせると共に、破格に対する注意を喚起し、将来臨床現場での心構えを示していくことを予定している。
