【背景・目的】抗がん剤治療は副作用が強いため、患者の体表面積や体重で補正した投与量で施行される。また、近年本邦では、種々生活習慣病の原因となる肥満が増えつつある。本研究では、体型の違いによるがん治療への影響を検討するため、副作用発現頻度に及ぼす肥満度の影響について検討を行った。【方法】2009年に京都大学医学部附属病院にて、脂溶性が高いパクリタキセルとカルボプラチンとの併用療法を行った患者101名を対象とした。末梢神経障害、白血球減少を調査項目とし、患者背景としては性別、年齢、肥満度、抗がん剤投与量、腎機能値(eGFR)、肝機能値(AST,ALT)、化学療法施行前の血球数を調査した。末梢神経障害における副作用関連因子を検討するためロジスティック回帰分析を行った。【結果・考察】末梢神経障害の発現について、BMIが18.5kg/m^2以下のやせ群では有意差は認められなかったものの(p=0.08)、オッズ比は0.24となった。また、パクリタキセルの投与量が1mg/m^2増えるとオッズ比は1.08となり、有意なリスク因子であることが示唆された。白血球減少の発現において、やせ(BMI 18.5kg/m^2以下)及び肥満(BMI 25kg/m^2以上)のオッズ比はそれぞれ0.40(p=0.17)、1.46(p=0.65)であった。また、ASTの増加(オッズ比1.14、p<0.05)、化学療法施行前の白血球数増加(オッズ比0.58、p<0.01)が有意なリスク因子であった。本検討から、統計的有意差は認められなかったものの、やせ型の患者では末梢神経障害が発現しにくい可能性が示唆された。

There were significant differences in the mean AST, AST and WBC counts before chemotherapy (P<0.05, P<0.01, P<0.14). The statistical significance of this study is to show the possibility of the occurrence of peripheral neurologic impairment in patients with multiple types of neurologic disorders.
