【研究目的】骨髄異形成症候群などの骨髄不全状態では造血幹細胞に原因不明の異常があり,アポトーシスが亢進していると推定される.また骨髄不全状態でのアポトーシスの割合の増加は,重要な予後因子と考えられている.本研究では,各種血球におけるアポトーシスの定量を行い,各種疾患による予後推定などの臨床的有用性を目的とする.【研究方法】疾患の対象として,白血病,骨髄異形成症候群で行なった.検体サンプルをあらかじめアポトーシスを誘導試薬(C_2-セラミド20μM)で15分反応後,目的細胞抗原,Annexin Vの染色しサンプルをフローサイトメーター(FCM)にて測定,解析した.【研究結果】1.健常者10名において好中球およびリンパ球におけるAnnexin Vの測定結果はそれぞれ26.0±2.7%,11.4±4.0%であった.2.骨髄異形成症候群(MDS)20名(RA15名,RCMD2名,RARS1名,RAEB2名)において好中球におけるAnnexin Vの測定結果は54.3±8.4%と健常者好中球と比較し高値傾向を示した.しかしMDSの病型による傾向は今回の結果では認めなかった.3.急性白血病11名(AML7名:内M1;3名,M4;2名,M5;2名,ALL4名:内B-ALL;2名,T-ALL;2名)において芽球におけるAnnexin Vの測定結果は11.1±3.1%で健常者リンパ球と同等の値を示した.また病型による傾向は今回の結果では認めなかった.4.今回の検討よりMDSにおける好中球のAnnexin Vの測定は健常者に比し高値傾向を示た.これはMDSにおいてアポトーシスの亢進状態を示しているためと考えられた.以上よりAnnexin Vの測定はMDSの診断の有用な1指標と成りうると考えられた.また急性白血病においては今後,症例を追加し,また臨床経過をあわせ検討していく予定である.
