人工内耳(CI)は、高度難聴者に対する医療として一般化しているが、成績に個人差が大きく、読話や手話を併用している例も多い。しかし、CI装用による情報伝達について、聴覚入力と視覚入力の両方から検討した報告は少ない。そこで本研究ではCI装用者の聴覚理解力と読話理解力とを利用した検査バッテリーを作成し、装用者における情報処理の特異性の解明を目的とした。対象はCI装用後1年以上が経過した中途失聴の成人CI装用者30名と小児CI装用者40名、健聴成人10名と健聴小児11名とした。母音の音声と口形画像を用い、視聴覚一致、視聴覚不一致、聴覚のみの3条件で提示した。被検者にはどの母音が聴こえたか判断するよう指示し、正答率と応答時間を測定した。その結果、成人、小児とも装用者の正答率は健聴者より有意に低く、応答時間は有意に長かった。また、装用者では視聴覚不一致条件は他の条件より有意に正答率が低く、応答時間は有意に長かった。成人装用者では語音聴取能と視聴覚不一致条件正答率の間に有意な相関を認めた。小児装用者を就学前のコミュニケーションモードで2群に分けて検討したところ、聴覚口話群は成人と同様に語音聴取能と視聴覚不一致条件正答率の間に有意な相関を認めたが、トータルコミュニケーション群では相関を認めなかった。本研究で作成した検査は健聴者にとっては非常に容易だが、CI装用者では特異な応答パタンを示し、視覚情報への依存の度合と聴取能との関連が示された。視覚情報も併用する装用者においては、健聴者では容易に検出できる視聴覚不一致条件でも、情報の整合性を検討する過程に時間を要すると考えられた。また小児では、就学前に聴覚口話で療育を受けた装用者の視聴覚情報処理は、聴覚を通して言語獲得した中途失聴成人に近いことが示唆された。
