【研究目的】本研究では,高血圧症の早期診断と予防医学における臨床検査の役割として,「高血圧治療ガイドライン2009」に取り上げられている尿中微量アルブミンに代わる腎機能障害の早期診断が可能な保険適応内の検査法を探る.特に,日常検査で簡便かつ,安価である尿沈渣検査による各種円柱の検出意義に着目する。各種円柱の検出頻度を高血圧症患者の臨床像と比較検討することで心血管イベントのリスク評価に役立つ可能性を検討する.【研究方法】(1)対象症例から得られた尿検体について尿定性検査,尿化学定量,尿沈渣検査を実施する.(2)(1)の測定結果との関連性について調べ,特に尿微量アルブミンと尿沈渣結果から得られる硝子円柱について比較検討を用いて詳細に検討する.(3)大規模な縦断的研究の実施の準備をする.東大病院腎臓内分泌内科外来患者のうち高血圧症患者から得られた尿検体を使用する.【研究成果】(1)尿沈渣検査における硝子円柱の検出.日常検査では,尿沈渣検査は,顕微鏡検査と自動分析装置により測定に大別される.両者を比較すると自動分析装置では,明らかに検出が低値であった.(2)尿蛋白定性の陰性群における硝子円柱の検出.尿微量アルブミン陽性検体では,顕微鏡検査にて硝子円柱を52%検出した.(3)高血圧症患者における硝子円柱の検出.尿蛋白定性の陰性群では,顕微鏡検査にて硝子円柱を40%検出,尿蛋白定性の陽性群では,75%検出した.【結語】高血圧症患者における硝子円柱の検出は腎機能障害の早期診断に繋がる可能性がある.

[Objective] This study aims to explore the possibility of early diagnosis of renal dysfunction by clinical examination in the early diagnosis and preventive medicine of hypertension. In particular, the daily examination is simple and safe, and the urine sediment examination is important for the detection of various parameters. The frequency of detection of various plaques is discussed in the clinical manifestations and comparative analysis of patients with hypertension, and the possibility of establishment of plaques in the critical evaluation of cardiovascular events is discussed. [Methods](1) Qualitative urine test, quantitative urine chemistry test, and urine residue test were performed for each patient. (2) The correlation between the results of (1) and the results of (2) is discussed in detail, especially the results of urine microanalysis and urine sediment analysis. (3)Preparation for the implementation of large-scale research. Department of Renal Endocrinology, Tokyo University Hospital. [Results](1) Detection of nitrate column in urine residue examination. Daily inspection, urine sediment inspection, microscopic inspection and automatic analysis equipment for determination. The automatic analysis device is used to compare the difference between the two. (2)Urine protein qualitative negative group detection of nitrate column. Microscopic examination revealed 52% of the nitrate columns detected. (3)High blood pressure patients in the event of a breakdown of the nitrate column. Urine protein qualitative negative group was negative, while urine protein qualitative positive group was negative, 40% and 75% respectively.
