【研究目的】ダウン症に摂食・嚥下トラブルや栄養障害、胃腸・消化器系の問題が多く、これらは低緊張による"丸のみ"や"舌前突嚥下"といった口腔運動の異常が原因として挙げられ、ダウン症の80%に認められるという報告がある。口腔運動の発達過程は、姿勢変換及び姿勢保持能力の発達など運動機能との統合的な発達を経て摂食機能として完成される。座位獲得から独歩までに1年以上必要とするダウン症児はその間に口唇閉鎖機能を獲得しないまま手づかみ食べや顎の運動が先行して発達する可能性があることが判明している。今回の研究目的は、以下の2点である。1.口唇閉鎖機能の獲得可能な時期(運動機能の発達との関連)が存在するのかを検証する2.粗大運動発達だけでなく姿勢変換及び姿勢保持能力といった視点から運動機能の発達の質を評価した上で口腔機能を評価していく必要性を提議する。【研究方法】あいち小児保健医療総合センター及び倉敷成人病センターを受診しているダウン症の乳児を対象とする。理学療法士による介入及び運動発達評価(PT評価)と言語聴覚士による介入及び摂食機能評価(ST評価)を1回/月の頻度で行う。介入においては、PT評価及びST評価を参考に母親へ食事に関する指導を行った。【研究結果】研究に同意が得られ、研究期間中に評価及び情報収集が可能であった4名について解析を行った。いずれの症例も離乳食期にあり、すべて男児であった。研究期間終了時点において、口唇閉鎖は獲得し、摂食における問題行動である舌前突嚥下は出現していなかった。運動発達においては症例毎による違いがみられるものの、全ての症例において独歩は獲得しておらず、体軸内回旋の欠落した姿勢変換を呈している事が判明した。従来では離乳完了とされる年齢だが、食環境への介入により問題行動が観察されずに経過している。体軸内回旋の出現が、より高度な食形態の処理に必要な機能である可能性が示唆された。

Assessment of the quality of motor function development and maintenance of gross motor function. [Method] The child health care system and the adult disease system were examined.
