【研究目的】本研究では潰瘍性大腸炎(Ulcerative colitis : UC)患者において、拡大内視鏡観察で所見のある粘膜を採取し、炎症細胞浸潤の程度や消化管バリアの一端を担っているTight junction (TJ)の発現状態、炎症性サイトカインで誘導されるInducible nitric oxide synthase (iNOS)の程度を病理組織学的に評価し、内視鏡所見と結び付けることで、正確な活動期診断ができると考えられる。さらに臨床経過を追い、全ての相関性を検討し、最終的には生検組織を取らずに内視鏡所見のみで易再燃性を予測できるような再燃予測因子を探ることが目的である。【研究方法】対象は琉球大学医学部附属病院を通院しているUC患者で下部消化管内視鏡検査を施行した症例。拡大内視鏡による粘膜模様を分類し、粘膜模様別に増悪率や再燃率を検討する。また、病理組織学的評価として炎症細胞浸潤の程度や腺管構造の変化、免疫染色によるTJ発現異常の有無を検討し、増悪因子や再燃因子となるかを検討する。その際、同時期に行われたUC以外の下部消化管内視鏡検査を施行した症例をコントロール群とした。【研究成果】研究期間内で登録されたUC群5例(寛解期2例、軽症1例、中等症1例)とコントロール群5例を検討した。UC拡大粘膜模様は活動期に微細顆粒状またはサンゴ礁様所見を呈し、寛解期は絨毛状所見を認めた。病理学的には活動期に炎症細胞浸潤が強く、寛解期と比較して著明に上皮内でiNOSが染色されていた。コントロール群ではTJのOccludinがUC群と比較し強く染まり、iNOSはほとんど染まらなかった。以上より検討症例数が少なく予後予測因子を検討するには至らなかったが、UC群はコントロール群と比較しiNOSが強く関与し、TJに影響を及ぼしていると予測された。今後も症例数を増やし検討していく予定である。

[Objective] This study was conducted to investigate ulcerative colitis.(Ulcerative colitis : UC patients were diagnosed with inflammatory cell infiltration, mucosal biopsy, degree of inflammatory cell infiltration, development of Tight junction (TJ) at one end of the digestive tract, degree of Inducible nitric oxide synthase (iNOS) induced by inflammatory cell infiltration, histopathological evaluation, endoscopic findings, and pathological findings. Correct diagnosis of active period. The objective of this study is to investigate the correlation between clinical processes and the results of the study. [Methods] Lower gastrointestinal endoscopy was performed in UC patients admitted to the Ryukyu University Hospital. The classification, increase rate and reburning rate of mucosal models in large endoscopes are discussed. Pathological evaluation, degree of inflammatory cell infiltration, changes in ductal structure, immunostaining, presence or absence of TJ abnormalities, enhancement factors, and reburning factors At the same time, the lower gastrointestinal endoscopy was performed in the same period. [Results] During the study period, 5 cases of UC were registered (2 cases in remission period, 1 case in remission period, and 1 case in moderate period). UC large mucous membrane is fine granular in active phase and fluffy in release phase. Pathology: Inflammatory cell infiltration in active phase, iNOS staining in active phase TJ's Occludin is stronger than UC's, iNOS is stronger than UC's Occludin. The number of cases discussed above is less than that of the prediction factor. In the future, the number of cases will increase.
