【目的】糖尿病治療では、患者への継続した指導と治療に対する意識付けを徹底することが重要である。そのため薬剤師や医療スタッフが患者の状態や治療への理解度を把握し、それらの情報を基にした療養指導を実施することが不可欠である。そこで本研究では、適切な糖尿病療養指導を実施するにあたっての患者理解度の客観的な評価や情報共有化の推進を目的とした業務支援ツール(リストとカード)を作成し、その有用性を評価した。【方法】糖尿病療養指導のうち、実施すべき薬剤適正使用に関する項目を、投与前の混和、空打ち、投与部位の確認、投与時の保持、逆血予防、保管方法の6項目に絞り込み、リストとして薬剤師間で必須指導事項を共有し業務の標準化を行った。リストをもとに、確認すべきインスリン手技の手順を写真撮影し、視力障害のある患者でも理解し易いように、A4サイズに加工した「カード」を作成した。実際の療養指導の場面では、カードを紙芝居のように見せながら指導を行うことにより患者理解度の向上を図った。糖尿病性網膜症を有する患者への療養指導の効果について調査を実施し、薬剤師による指導介入の有用性について評価を行った。【成果】九州大学病院の眼科病棟において、糖尿病性網膜症を有しインスリンを使用している患者6名に対し薬剤師が指導介入したところ、3名(50%)に改善(自己注射補助器具の提案による使用感の向上、投与部位の分散化による注射部位の硬結回避、およびインスリン製剤変更の提案によるHbAlcの改善)が認められた。本調査により、糖尿病患者、特に網膜症を有する患者に対し、インスリンの適正使用継続に向け指導介入することは重要であると考えられた。本研究で作成した業務支援ツールを活用することによって、糖尿病患者に対し、理解し易く効果的な療養指導を行うことが可能になると示唆された。
