【目的】頚髄損傷におけるADL上の自立度を評価するのに、残存機能髄節高位が重要であることは言うまでもない。この点から、FranlelAとBに対してZancolli分類はその病態を把握しやすく有用である.しかし、FrankelA、Bと同様に歩行不可能であるFrankelCではZancolli分類を用いることが出来ず、その自立度をイメージしずらい。今回、American Spinal Injury Association(以下ASIA)のmotor scoreを用いFrankleCにおいてADLの自立度を評価する上で、その有用性をZancolli分類と比較しながら検討した。【対象】1993年から95年まで当院で入院治療し得た頚髄損傷FrankelA28例、B16例、C26例の計70例で、骨傷例は36例、損傷のない例は34例であった。性差は男性65例、女性5例で、受傷時年齢は12歳から69歳、平均42.2歳であった。【方法】麻酔高位判定は、FrankelA、B症例ではZancolli上肢残存運動機能分類を用い、左右差があった場合は良好な方を選択した。FrankleCでは、AISAのmotor scoreの上肢50点満点を5点毎に分け用い、FrankelA、B症例と比較検討した。次にADLとの関連は、麻酔レベルとADL上基本動作である食事動作・排尿動作・車椅子移乗動作について、各々自立度を介助の有無と方法で評価した。【結果・考察】Zancolli分類とASIAのmotor scoreとの間には高い相関を認め、Zancolli分類で表示し得ないFrankelCの評価の際に、ASIAの上肢のみのmotor scoreによるADL上の自立度判定は有用性があると思われた。ADLとZancolli分類、ASIAの上肢のみのmotor scoreとの間に高い相関が認められ、ADL上基本動作である排尿動作、車椅子移乗動作において各々C6AからC6B1と30点台に自立の境界があるものと思われた。FrankelCでのASIAの上肢のみのmotor scoreによるADL上の自立度判定は、FrankelA、BにおけるZancolli分類ほど自立度をイメージし易いわけではないが、ある程度その病態を把握でき有用であった。このことは、頚損患者をとりまく家族および医療従事者間におけるその病態の情報伝達に役立つものと思われる。
