目的および方法:股関節手術時に希釈式自己血輸血を行った8名(希釈群)と行わなかった9名(対照群)を対象として検討を行った。希釈は血液希釈液としてハイドロキシエチルスターチを用い、ヘマトクリット24%を目標に行った。1:手術前2-3日以内、2:希釈30分後、3:手術終了30分後、4:術後1日目、5:術後3日目の5測定点において検体採取を行い、トロンボエラストグラフィー、ヘマトクリット(Ht)、血小板数、フィブリノーゲン量、PAI-1(1:、4:、5:のみ)、Dダイマー(1:、4:、5:のみ)、クレアチニンクリアランス(CCr)、尿中N-Acethyl-beta-d-Glucosaminidase(NAG)等の測定を行った。結果ならびに考察:Htは希釈群37,23,30,31、対照群35,30,29,30,31と変化して2:において希釈群は対照群に比較して有意な低下を認めた。凝血塊強度の指標であるMA(mm)は希釈群44.9,37.8,43.7,45.3,55.0、対照群45.2,45.3,47.5,47.3,54.0と変化して希釈群は2:において対照群に比較して有意な低下を認めた。血栓性合併症の指標であるDダイマー(ng/ml)は希釈群113,928,395、対照群116,939,438と変化したが両群間に有意差はなかった。糸球体濾過量の指標であるCCr(ml/min)は希釈群95.1,62.5,78.99,115、対照群74.9,51.1,105,118,118と変化したが両群間に有意差はなかった。尿細管障害の指標であるNAGインデックス(U/I)は希釈群4.6,13.4,31.1,10.1,16.6、対照群4.3,12.4,16.8,10.4,17.0と変化したが有意差はなかった。結論:以上の結果より希釈式自己血輸血は術中の凝固能を有意に低下させるが術後の凝固能には有意な影響を与えなかった。また血栓性合併症を低下させる作用はなかった。腎機能に対する悪影響はなかった。

Objective: To study the effect of blood transfusion on femoral joint surgery. The index of urinary tubule damage is 4.6, 13.4, 31.1, 10.1, 16.6, and the corresponding group is 4.3, 12.4, 16.8, 10.4, 17.0. Conclusion: The above results indicate that the coagulation energy during operation is lower than that after operation. Thrombotic complications are reduced in function. The kidney function is affected.
