・前立腺肥大症による下部尿路閉塞である前立腺性閉塞を有する高齢男子を対象に排尿圧尿流率同時測定(Pressure-flow study)を施行し、結果をコンピュータ解析した。・対象は臨床的前立腺肥大症の46例で高圧排尿の指標として特に排尿開始圧(Pves. open)に注目し、Pves. openと治療前の排尿筋の活動性をはじめ自他覚所見との関係及び前立腺切除術後の所見の特徴につき検討した。・結果的にPves. openをはじめとする下部尿路の閉塞のパラメータが高値であるほど膀胱の収縮力(work factor)は増大しており、これらは膀胱内肉柱形成所見とも相関していた。また、これらは膀胱の無抑制収縮の陽性率とも相関していることがわかった。・これらの因子はいずれも患者の自覚症状のつよさとは相関を示さなかった。また、閉塞の程度の違いによる最大尿流率や残尿量の差を認めなかった。・前立腺切除術により閉塞を解除した症例群では、全例術後明らかに尿流や自覚症状の改善をみたが、高圧排尿の程度の強かった症例群では術後も若干高圧傾向にあった。・術前Pves. openが高値で閉塞のつよかった症例では、短期的に排尿筋の過活動性が残存するために、一時的に尿失禁など刺激症状の出現・増悪を呈した。・これら排尿筋の過活動性は長期的に徐々に減弱し、術前にみられた閉塞の程度によるwork factorの差も次第に認められなかった。・代償性の排尿筋過活動や収縮力増大が前立腺性閉塞のある患者の排尿効率を改善させており、それが閉塞の解除とともに徐々に正常化することが推察された。
