研究目的:習慣流産の拒絶反応の標的抗原を明らかにするために,胎児由来の胎盤に発現されているHLA-G抗原およびHLA-G抗原に結合するペプチドを規定するTAP(Transporter Associated with antigen Processing)遺伝子の多型性を習慣流産患者について検討した。研究内容:1.対象:当院で習慣流産のため夫リンパ球による免疫療法を施行した49組,98例を対象とした。検討内容:TAP遺伝子をPCR-RFLP法により多型性および夫婦間の遺伝子の共有率を解析した。研究方法:1.凍結顆粒球よりPhenol-Chloroform法により染色体DNAを抽出した。2.TAP1,TAP2遺伝子の多型部位をPCR法により増幅した。3.制限酵素(Sau3A,AccI,AccII,NspI,BfaI,RsaI)により切断し,TAP1A-1D,TAP2A-2Hを分類した。成績:1.TAP遺伝子頻度1)TAP遺伝子頻度:TAP1A 86.2%,1B 11.2%,1C 0.5%,1D 2.0%A:Ile-Asp,B:Val-Gly,C:Val-Asp,D:Ile-Gly2)TAP2遺伝子頻度:TAP2A 54.4%,2B 21.2%,2C 13.2%,2D 3.0%,2E 4.1%,2F 0%,2G 0.5%,2H 3.6%A:Val-Ala-Thr-Stop,B:Val-Ala-Ala-Gln,C:Ile-Ala-Thr-Stop,D:Ile-Thr-Thr-Stop,E:Val-Thr-Thr-Stop,F:Ile-Thr-Ala-Gln,G:Val-Thr-Ala-Gln,H:Ile-Ala-Ala-Gln2.夫婦間のTAP遺伝子の共有頻度1)TAP1遺伝子の共有頻度:共有なし:0%,共有数1:46.7%,2:53.3%1)TAP2遺伝子の共有頻度:共有なし:32.1%,共有数1:39.7%,2:28.2%考察:習慣流産夫婦におけるTAP遺伝子頻度は諸家の健常人のものと同等の頻度であった。しかし,健常夫婦間におけるTAP遺伝子の共有率と習慣流産との頻度差やHLA-G抗原の多型性に関しては今後検討を要す。

Objective: To investigate the polymorphism of TAP (Transporter Associated with Antigen Processing) gene in patients with habitual abortion. Study contents: 1. Target: 49 groups of patients were treated with immunotherapy for habitual abortion, 98 cases were treated with immunotherapy. Content of the discussion: Analysis of polymorphism of TAP gene by PCR-RFLP and common rate of gene between couples. Methods: 1. Chromosome DNA extraction from frozen granulocytes by Phenol-Chloroform method. 2. Amplification of polymorphic sites of TAP1 and TAP2 genes by PCR 3. Restriction enzymes (Sau 3A, AccI, AccII, NspI, BfaI, RsaI) are classified into four groups: TAP 1A-1D, TAP 2A-2H. Results: 1) TAP gene frequency: TAP 1A 86.2%, 1B 11.2%, 1C 0.5%, 1D 2.0%A: Ile-Asp, B: Val-Gly, C: Val-Asp, D: Ile-Gly 2) TAP 2 Genetic Frequency: TAP 2A 54.4%, 2B 21.2%, 2C 13.2%, 2D 3.0%, 2E 4.1%, 2F 0%, 2G 0.5%, 2H 3.6%A: Val-Ala-Thr-Stop, B: Val-Ala-Ala-Gln, C: Ile-Ala-Thr-Stop, D: Ile-Thr-Thr-Stop, E: Val-Thr-Thr-Stop, F: Ile-Thr-Ala-Gln, G: Val-Thr-Ala-Gln, H: Ile-Ala-Ala-Gln 2. Common frequency of TAP inheritance between husband and wife 1) Common frequency of TAP 1 inheritance: Common frequency: 0%, Common number 1: 46.7%,2: 53.3%1) Common frequency of TAP2 gene: Common frequency: 32.1%, total frequency: 1: 39.7%,2: 28.2%Investigation: The frequency of abortion is similar to that of healthy couples. However, the relationship between the common rate of TAP genes, the difference in frequency of habitual abortions, and the polymorphism of HLA-G antigens among healthy couples will be discussed in the future.
