本研究は,持続的圧力下の義歯床下粘膜上皮細胞に惹起される増殖活性の変化に対して糖尿病が与える影響について,AgNOR染色によって観察されるNORs数を指標として定量的に検討することを目的とした.実験動物にはストレプトゾトシンによって糖尿病を誘発した15週齢のウイスター系雄性ラット175匹(1群35匹×5群)を用いた.臼歯部口蓋を対象として,義歯床下組織に対して0.0,0.5,1.5および3.4kPaの持続的圧力を加える可撤性の義歯床を1群ずつの動物に装着し(無圧群,0.5,1および3.4kPa加圧群),残る1群の動物は義歯床を装着することなく経過させた(義歯非装着群).義歯床下粘膜および義歯床の清掃は,3〜4日毎に行った.義歯床装着の3日,1,2,4,8,12および20週後に各群の5匹ずつから採取した口蓋粘膜は通法に従って2μmのパラフィン切片とし,これにAgNOR染色を施して基底細胞の細胞核1個あたりの平均NORs数を各実験群毎に求めた.各観察期間毎に各実験群間の有意差検定(Mann-Whitney U-test)を行った.その結果,NORs数は,義歯非装着群ならびに無圧群では観察期間を通じて両者が同様でほぼ一定の値を示した.また,0.5kPa加圧群では義歯床装着1週後に,1.5および3.4kPa加圧群では3日後以降8週後に至るまで,それぞれがいずれも無圧群に比べて有意に小さい値を示し,持続的圧力の設定値の増加に応じた減少を示した.しかし,0.5kPa加圧群では2週後以降,1.5および3.4kPa加圧群では12週後以降は無圧群との間に有意差を示さなかった.本研究において得られた糖尿病ラットにおけるNORs数は,本研究と同一条件下で観察した健常ラットのそれに比べて小さい値を示し,加圧群におけるNORs数の減少程度は,糖尿病の発症と持続的圧力による影響をほぼ相加的に受けたものであった.以上のように,NORs数を指標とした持続的圧力下の義歯床下粘膜上皮細胞の増殖活性は,糖尿病の発症自体と持続的圧力の両者の影響を受けて相加的に低下するに止まり,糖尿病の影響をほとんど受けなかった.

This study aimed to investigate the effects of sustained stress on the proliferation of epithelial cells in subgingival mucosa on diabetes mellitus, and to quantify the number of NORs detected by AgNOR staining. In the 15-week-old group, 175 males (1 group of 35 ×5 groups) were used. A group of animals were installed on the prosthetic dental bed (no pressure group, 0.5 kPa, 1 kPa and 3.4 kPa pressure group), and a group of animals were installed on the prosthetic dental bed (no pressure group, 0.5 kPa and 1 kPa pressure group). The cleaning of the lower mucous membrane of the dental bed is carried out every 3 to 4 days. After 3 days, 1, 2, 4, 8, 12, and 20 weeks, 5 samples of each group were taken by the mucosal passage method, and 2μm sections of each group were obtained by AgNOR staining. The average number of NORs in the nuclei of basal cells was 1. Mann-Whitney U-test was performed during each observation period. As a result, the number of NORs is equal to the number of non-loaded groups and the number of non-loaded groups. The pressure of 0.5 kPa is increased after 1 week, and the pressure of 1.5 kPa is increased after 3 days, and the pressure of 3.4 kPa is decreased after 8 weeks. After 2 weeks, the pressure of 0.5 kPa was reduced, and after 12 weeks, the pressure of 1.5 kPa was reduced. In this study, the number of NORs in diabetes mellitus was determined. Under the same conditions, the number of NORs in diabetes mellitus was determined. In addition, the number of NORs in the above indicators decreased significantly under sustained stress, and the growth activity of epithelial cells in the subgingival mucosa decreased significantly under sustained stress.
