21世紀には超高齢化社会を迎える我が国では、国民の健康への関心がますます高まり、中でもよく"咬む"ことの大切さが新聞、ラジオ等で報道されている。しかしながら、歯科医療の現場を見てみると、"どれくらい咬めているのか"という咀嚼能力の評価は未だ患者サイドの主観に頼ることが多く、診断や治療効果における咀嚼能力の客観的評価の確立が望まれている。本研究では、補綴物の治療効果の1例として、中でも近年増加傾向にあるインプラント補綴に着目し、部分床義歯との比較を行うことを目的とした。被験者は、65歳以上の自覚的にも他覚的にも疾病のない健康な高齢者で、咀嚼能力測定法としては、臨床応用に適し物理的にも非常に安定感がある人工試料カプセルを用い、ゴムカプセル内に封入された色素内包顆粒が外力で破壊され色素が漏出してくることから、その濃淡により咀嚼粉砕能力をエネルギーで表示する方法を用いた。この方法は回収率も高く100%であり、測定方法においもて被験者を前にしてリアルタイム(1〜2分)で測定結果を知ることができるなどの多くの利点を持つことから、我々はこの人工試料カプセルに注目し、インプラント装着者用としては基本の硬度カプセルを、部分床義歯装着者用としては基本の約1/2硬度の軟性カプセルを製作し、各被験者の同意を得て、部分床義歯装着時とインプラント補綴物装着時における咀嚼能力の測定を行った。今後、データ数を増やし、得られたデータの分析から、部分床義歯装着時に比べインプラント補綴物装着時には、どの程度咀嚼能力の向上が見られたかを把握し、補綴物の治療効果の1例としてのインプラント補綴物の場合についての客観的評価を確立する予定である。
