[目的]顎口腔機能の加齢現症を,上下歯列を一対とした一口腔単位で観察し,下顎前歯部の歯軸変化に及ぼす影響を機能的,配置的診査から病態統計学的に検討することを目的とした.[方法]補綴治療を希望して当科来院した患者のうち,下顎6前歯が全て補綴されずに残存している患者100名(22歳〜81歳)を対象に,プロトコールの記載および診断用模型作製の印象採得,模型分析を行いデータベース化を行った.また下顎前歯部の歯軸の状態については,今回の調査のために,新たな分類基準を設定し,パーソナルコンピューターのモニター上に,市販のソフト(安永コンピューターシステム)を用いて模型の画像を取り込み解析を行った.[結果]1.下顎前歯部歯軸変化は,形態的特徴から大きく9分類(平担型,凹型,凸型,凹凸型,空隙型,叢生型,右上り型,左上り型,唇舌側傾斜型)され,更にその混合型も認められた.2.歯軸の変化の基準を,左右に犬歯歯頚部中央部を通る水平面と左右中切歯中央を通る垂直面に設定することで,臨床的に平易に規格化された模型分析が可能であった.3.上下歯列の対向関係をEichnerの分類からみると,A群では若年層が多いが,年齢が高くなると,B群C群が多くなる傾向が認められた.B群では,固定式と可撤式の補綴物の併用の症例が大多数であるのに対し,C群では全てが可撤式であった.また歯科治療の放置症例も認められた.4.年齢,歯科的既住と補綴方法の違い,予後などによって,歯軸変化や口腔内の病理像に違いが認められたが,一定の傾向は認められなかった.今回の被験者は,特に60代が多かったために,年齢的な変化の検討が困難であったが,いわゆる8020に相当する被験者は,歯軸変化が少なく,良好な咬合支持が保たれていた.今後,各年齢層の被験者数を増やして,検討していく予定である.

In the past, dental treatment and repair methods have been violated, and the dental axis has been changed and the pathological images in the oral cavity have been violated. Today, the 60th generation of the quilt is more difficult to discuss, and the 8020 is equivalent to the quilt, the axis is less, and the bite support is maintained. From now on, the number of victims in each layer will increase, and the number of victims will be determined.
