全身疾患のため出血傾向にある患者はブラッシングを控える傾向にあるが、プラーク細菌の局所感染のために生じる歯肉出血は、適切なプラークコントロールが行われない限り改善されずに炎症は増悪し、いっそう患者の口腔内清掃意欲を消失させていると危惧される。そこで今回、専門家による歯周管理の必要性を明らかにすることを目的に、血小板減少症患者を対象に機械的清掃を主とした初期治療を行い、治療前後の各種臨床パラメーターおよび歯肉溝浸出液量を比較してその有効性についての検討を試みた。1.対象者12名の平均血小板数は35,667/μlであり、うち9名は特発性血小板減少性紫斑病であった。2.初診時のアンケートより、患者(12人中8人)はブラッシング時の出血や歯肉腫脹を自覚しながらも歯科治療に対し消極的であったと考えられたが、清掃指導開始後は意欲的に受診する傾向がみられた。3.初期治療開始後、PCR,GI,BOPともに改善傾向を認めた。4.歯肉溝浸出液量は、炎症部位では平均0.85μlであり健常部位の平均0.28μlに対して有意に高く、歯肉の炎症との関連がみられた。以上より、機械的清掃を主とした初期治療が出血傾向患者の歯肉の炎症改善に有効であることが示唆された。また、患者の歯肉出血に対する不安を取り除き自己の健康管理に対する認識を高めたことがアンケートより推察された。しかし、全身状態が悪化して外来診療の継続が困難となった例もあり(1名は無菌室入室となり、舌潰瘍および難治性口内炎のケアのため定期的に往診を続けている)、歯科受診に対する動機づけが困難なこともあって期待した症例数を得られていない。本研究は隣接する原医研内科の協力を得ており、この連携を機に今後さらに各種パラメーターの有用性の検討も含めて長期的な観察が必要と考えている。

The number of cases of difficulty in external diagnosis and treatment (1 case of sterile room entry, 1 case of tongue ulcer and 1 case of failure of treatable endostomatitis) and the number of cases of difficulty in dental diagnosis and treatment were obtained. This study is aimed at exploring the usefulness of the collaboration between the Department of Medicine and the Department of Health, including long-term research.
