昨年度、高血圧患者を対象に降圧剤が骨代謝に与える影響に関する横断研究を行った。その結果、降圧剤未使用の高血圧患者と、降圧剤投与して治療した高血圧患者とでは後者のほうがカルシウム代謝ホルモンであるPTHintが有意に低値を示した。今年度は未治療の高血圧患者を対象として、降圧剤を開始した前後での骨代謝変化に関する2ヶ月間の前向き研究を行った。対象は当院初診の高血圧患者のうち合併症のないものとした。方法は降圧剤開始時、1ヶ月後、2ヶ月後の3回で骨代謝・カルシウム代謝マーカー(PTHint、BGP、1.25(OH)2D3、ALP、デオキシピリジノリン;DPD/Cr)を測定し、検討した。男性8人、女性3人の11人が対象となったが男女各々3人ずつが脱落(2人は血圧コントロール不良で2剤併用となり、4人は自己判断で外来通院を中止)したため症例は5例となった。結果は、降圧剤開始後2ヶ月の時点において、1例が6%の増加を示したが、他の4例は23〜50%という大幅な減少を示した。この結果は昨年度の横断研究における結果とほぼ一致する。また、骨吸収マーカーであるデオキシピリジノリンは5例全例において、降圧開始後2ヶ月で8〜46%の減少を示した。以上のことから、高血圧患者に於いては降圧剤を投与して血圧を下げると骨吸収が抑制される可能性があることが示唆された。また、低骨塩量患者の各種治療効果を超音波骨密度測定装置にて経過観察しているが、治療が途中で変更になった例や治療脱落が多く、まだ評価するに至っていない。
