一側肺動脈閉塞試験;Unilateral pulmonary artery occulusion tcst(UPAO)によって及ぼされる血管作働物質の変化を心肺循論動態因子の変動と同時に測定し,更に,術後の実測値との関係についても検討した. (方法)肺切除予定患者25名(肺全摘術予定患者10名(P群),肺葉切除予定患者15名(L群))を対象とした.術前に経大腿静脈的に一側肺動脈閉塞試験を施行し,閉塞前,閉塞直後,閉塞10分後,15分後,閉塞解除後の大腿動脈血を採血し,血管拡張因子と収縮因子をラジオイムノアッセイ法で測定した.また,同採血時の心肺循環動態を監視した,さらに肺切除後急性期の変化はSwan-Ganz cathclcrを用いて測定した.(結果)男性19例,女性6例.平均年齢64.7歳(45〜74歳)であった.UPAO後のmPAPは閉塞直後から有意に上昇し,解除後には改善傾向にあったが依然として高値を示した.また,COは不変であったが,TPVRは閉塞後有意に上昇した.一方,血管拡張因子(血漿ANP,AM値)及び血管収縮因子(血漿ET-1値)は閉塞後共に不変であった.更に,肺切除後の変化を測定したところ術後mPAPはP群のみ術直後から有意に上昇したが,L群とは有意差はなく,術後3日目のみP群が有意に高値を示した.さらに,TPVRはP群の術直後のみ,術前に比べ有意に上昇したが,その後は術前後間には差はみられなかった.しかし,P群とL群との比較では術直後から術後3日目までの全観察期間有意にP群が高値を呈した.また,術後の血管拡張因子の変化ではANPが術直後から上昇し,L群は術後時間経過とともに改善傾向にあるのに対し,p群は有意に遷延傾向にあった.AMはL群は不変であり,P群のみ術後,高値を示した.一方,血漿ET-1値はP群,L群共に有意な変動は無かった. (考察)UPAOでの肺循環動態因子の変化と血管収縮因子及び血管拡張因子の変化は異なった.また,肺切除後は肺葉切除群と肺全摘群とは術後の循環動態の変動および,血管拡張因子との変化において有意差がみられ,肺血管床の残存容量により心房系のみならず心室系への負荷が生じるがこのことを術前のUPAOでの予測は困難である事が示唆された.
