癒着性中耳炎の耳管機能について耳管機能検査法による検討を行ったところ、耳管の開大がみられるものが多かったが、健常者に比較すると抑制されていた。つぎに中耳腔のガス換気の状態を評価するために、癒着性中耳炎で換気チューブを留置した症例について、その中耳腔全圧を手術後に経過を追って観察した。滲出性中耳炎では中耳腔全圧は半年以降で粘膜の腫脹がとれる頃に一致して上昇する傾向を示したが、癒着性中耳炎では全圧は半年以降でもその上昇が悪く、中耳粘膜の換気状態が改善されにくいものと推測された。また癒着性中耳炎術後の再癒着の原因として、術中の骨壁の露出も一つの要因と考えられている。そこで正常成熟家免を用いて、可及的に鼓膜へ損傷を与えないように鼓室及びbulla粘膜を除去、掻爬し、長期に渡って経過を観察した。鼓膜は弛緩部を中心とする陥凹、混濁を認めた。組織を観察すると、粘膜掻爬後の骨面上に新生骨の増生が観察され、その周囲に結合組織の増生を認めた。一部に岬角から鼓膜への連続性を認める結合組織も認められ、今後癒着性変化を形成する可能性も考えられた。このように癒着性中耳炎の成因には耳管機能障害特に陽圧、陰圧解除不能、中耳粘膜の換気障害が関与していると考えられ、術後癒着の成因としては術中の粘膜掻爬の影響も関与していると考えられた。
