杏林大学附属病院眼科緑内障外来を受診した患者から原発閉塞隅角緑内障(PACG)、狭隅角眼、Plateau iris syndrome(PIS)と診断された患者を選択し、ハンフリー社製Ultrasound Biomicroscope model 840(UBM)を用い隅角の生体観察および記録を行った。解析可能であった症例は9例15眼(男性1例2眼、女性8例13眼)であり、年齢は33歳から75歳、平均57.2歳であった。内訳はPACG急性発作の僚眼2例2眼、慢性原発閉塞隅角緑内障2例2眼、狭隅角眼5例10眼、PIS 2例3眼であった。PACG、狭隅角眼ではレーザー周辺虹彩切開術(LI)以前の状態を、PISはLI後でレーザー隅角形成術以前の状態を対象とした。また、狭隅角眼は暗所における自然散瞳により、少なくとも1ヶ所で隅角閉塞が確認されたものを対象とした。上方、下方、耳側、鼻側の4方向での垂直断層像を記録した。画像から石川らの方法に準じArea0からArea3までの面積を測定、合計をその部位の隅角面積とし、各部位の合計面積の平均を対象眼の隅角の広さとして評価した。面積の測定には我々とウエルシステムで共同開発したUBM専用画像解析ソフト「RATE」を使用した。全体の平均隅角面積は0.076mm^2であり、石川らによる開放隅角眼の測定結果0.427mm^2と比較すると有意に狭かった。今団測定した3群ではPACGが0.066から0.120mm^2、平均0.097mm^2と最も広く、狭隅角眼では0.056から0.112mm^2、平均0.076mm^2、PISでは0.013から0.062mm^2、平均0.043mm^2と最も狭かった。症例数も少なく、比較は困難であるが実際の緑内障発症機転には単に隅角の広狭のみでなく、様々な因子が関与していることが予測された。角膜径、眼軸長、水晶体厚、虹彩厚、虹彩水晶体接触距離等の因子を含めて更なる検討が必要と考えられた。
