项目摘要

大分県下の障害児施設と連携し、各施設で療育中の重症身障児の中で胃食道逆流症(GER)の疑われる症例に対し、臨床症状、管理状況の調査およびGERの検査による評価を行った。スクリーニングとしてpHモニタリングを、主に当該施設の通常の管理下にて行い、大分医大での解析後、さらに疑診例に対し消化管造影検査、内圧検査を行った。治療方針については合同のカンファレンスにて決定した。2年間の検討で2施設の2例がpH<4の時間率:15.8%(13歳男性)および11.8%(41歳男性)と手術適応に該当し腹腔鏡下噴門形成術を行った。また他の2例に単独の胃瘻造設術を行った。噴門形成手術に際し、強度の脊椎変形による体型および噴門部の位置異常からワーキングスペースが狭く鉗子類の相互干渉が問題となり、手術体位とtrocarの部位設定に工夫を要した。両者とも肺合併症を有し全身状態も不良であったが、通常の8-10mmHgの気腹下で周術期の合併症は見られず、各種モニタリングにも異常なく、術後の早期、中期のQOLも良好であった。療育者のQOLも良好となり、訪問による懇談、教育講演と合わせ手術療法についての理解も進んできている。今後スクリーニングを継続するとともに、長期的な視点で手術適応例、疑診例を含め各症例における病態の変化を追跡する予定である。

Examples and cases of suspected diagnosis include cases of various symptoms, and the pathological change of the disease is traced and predetermined.
