咬合高径の臨床的な決定法として安静位空隙利用法、嚥下運動利用法、顔面計測法、発音利用法などがあるが、いずれも確実な方法でなく、これらの決定法を用いて総合的な観点から咬合高径を決定すべきであるといわれている。最近、適切な咬合高径では上下唇の面積が一致するという臨床報告がなされているが、定量的に評価した報告はみあたらない。そこで、適切な咬合高径を有する成人有歯顎者の上下唇の面積が一致するかどうか、また、年齢による差異の有無を調べることにした。前年度は、20歳代20名と30歳代20名の合計40名の成人有歯顎者の上下口唇の面積を調べたが、本年度は、成人有歯顎者40名(40歳代20名、50歳代20名)を被験者として選択し、中心咬合位における正貌の上下口唇の各面積を調べた。結果は、以下の通りである。1. 40歳代20名と50歳代20名の合計40名の成人有歯顎者を被験者として選択した。被験者の基準は、全身と咀嚼系に臨床的な異常が認められず、第3大臼歯を除く天然歯列を有し、上下第1大臼歯の近遠心関係がAngle I級の咬合状態であり、適正な前歯被蓋を有する者とした。2. 40歳代20名と50歳代20名の上口唇の面積は、どちらも個人差が認められた。3. 40歳代20名と50歳代20名の下口唇の面積は、上口唇と同様にどちらも個人差が認められた。4. 前年度の被験者40名(20歳代20名と30歳代20名)と本年度の被験者40名(40歳代20名と50歳代20名)の合計80名について、上下口唇の各面積を上下口唇間で比較した結果,各被験者ともにほぼ同じ値を示し,両者間に有意差が認められなかった。また、各年代毎に上下口唇の面積を比較しても両者間に有意差が認められなかった。5. これらの結果から,正常有歯顎者の中心咬合位における前頭面で見た上下口唇の各面積は,個人差があるものの,同一個人ではほぼ同じであり,この観察が咬合高径の解剖学的指標になる可能性が示唆された。

The clinical determination of occlusal height is based on the use of quiet space, swallowing movement, facial measurement, and sound. For adults with teeth, the area of the upper and lower lips is the same, and the difference between the upper and lower lips is the same. The area of upper and lower lips of 40 adults with teeth was adjusted in the previous year (20 in the 20 th generation and 20 in the 30 th generation), and the area of upper and lower lips of 40 adults with teeth (20 in the 40 th generation and 20 in the 50 th generation) was adjusted in the current year (20 in the 40 th generation and 20 in the 50 th generation).
