顎口腔領域悪性腫瘍の転移リンパ節に対する術前放射線治療、化学療法の効果を病理組織学的に検討し、US所見と対比した。対象は24症例の43リンパ節で、大星らの組織学的治療効果度を用いて判定したところ、19リンパ節がGrade(G)IIa、3リンパ節がGIIb、6リンパ節がGIII、15リンパ節がGIVであった。GIIb、III、IVを効果あり群、GIIaを効果なし群としUS所見と比較検討したところ、効果あり群では治療後に縦径、横径が減少、境界不明瞭、辺縁不整を示すリンパ節が増加していたが、効果なし群では治療前後ですべての項目において統計学的な有意差は認められなかった。また、放射線・化学療法前の超音波所見を2群間で比較した結果、効果あり群では辺縁平滑、内部エコー低、リンパ節内の血流を示したリンパ節が多く、効果なし群では辺縁不整、リンパ節境界部の血流を示したものが多かった。縦径、横径においては統計学的な有意差が認められなかったことから、リンパ節の大きさと治療効果の有無には関連性がなかったことが示唆された。以上の結果を下記の学会で報告した。「遊佐 浩、他:口腔扁平上皮癌の転移リンパ節に対する縮小率と組織学的効果判定との関係.第44回日本口腔外科学会総会(1999年10月8日、東京)」また、下記の論文をthe Ultrasound in Medicine&Biologyに投稿中である。「Hiroshi Yusa,et al.:Ultrasonographic assessment for response to rediochemotherapy of metastatic cervical lymph nodes in head and neck cancer.」
