项目摘要

乳房切除による変化が患者と家族に及ぼす影響について、患者インタビューによって検討を行ってきたが、今年度は焦点を絞り、乳房切除とそれを及ぼしたがんという疾患が、患者と家族によってどのように扱われているかについて調査した。患者会に所属している乳がん体験者及び、外来でフォローアップを受けている乳房切除患者を対象としたインタビュー及びその質的分析により、以下のことが明らかになった。患者は、乳がんであることへの思いと乳房を切除したことについて、家族と語り合うことは好まなかった。別に話してもかまわない、と述べつつ実際に駆られるまでは、その話題に患者と家族双方でふれないように努めていた。しかし、患者は家族が自分を気遣ってくれている、わかってくれていると感じており、それは"言葉にできない"か、さりげない"どうだ?"という問いかけに依っていた。深く語り合ったり、不安やつらさを訴えない理由として患者は、夫は男の人だから、息子は男の上若いから、娘は未だ若いから、親はもう高齢であるから、自分の気持ちや大変さはわからないのだ、と述べた。このように思いや辛さは家族とは共有されないことが多かったが、患者は頼りは家族だと述べた。現実の乳がんについては、患者は診察や検査の結果を家族と共有していたが、外来受診はほとんどの患者が一人で来院し、家族への情報提供は患者自身の判断により、調整されていた。つまり患者はできる限り自分自身で病気とその影響に対処していた。この結果より、乳がん患者は自分の病気への思いなどは家庭内に持ち込まないよにし、家族を乳がんによる影響を遠ざけようとし、家族も患者ががんであることや身体的変化についてはふれない、と言うことで家族内のバランスが保たれていたことが明らかになった。
