医療現場での応用をめざしたガン治療における情報希求度の検討

项目摘要

1.目的:(1)疾病に対する情報希求度を測定する質問紙を作成し、情報希求度に影響を与える要因を明確にすることと、(2)ガン患者の情報希求度が、情報を提示された際の満足度にどのように影響するかを検討することであった。2.方法(1)対象:無作為抽出された関東地方在住の16歳から79歳までの男女316名と、ガン治療のため現在入院中の患者26名であった。(2)質問紙:「情報希求度」「自己決定度」「Health Locus of Control」「疾病の際のストレス対処法」「疾病の際の情動状態」「非理性的な信念」「属性(年齢・性別・疾病の既往歴等)」から成っていた。3.結果と考察:(1)「情報希求度」24項目の因子分析を行った結果、3因子18項目が抽出された。これは先行研究(大木・福原,1997,1999)とほぼ同様の結果であり、交差的妥当性が検証された。また各因子のα係数も高く、信頼性も検証された。過去に罹患した疾病の重症度による情報希求度の差は認められず、現在疾病に罹患している人の情報希求度を測定するのに妥当な質問紙であることが示された。(2)基準関連妥当性を検証するため、ガン患者と面接を行って情報希求度を判定した。面接結果と質問紙の情報希求度間に高い相関係数が得られたため、基準関連妥当性が示唆された。しかし人数が少ないためさらなる検討が必要とされた。(3)末期ガンのケースにおける情報希求度を基準変数、他の尺度や属性を説明変数とした重回帰分析の結果、脳卒中とマヒのケースの情報希求度と性別等が選択された。(4)ガン患者に対して施行した結果をもとに、医師に対して説明の量や内容の助言を行った。面接の結果、このような介入を行った患者の医師の説明に対する満足度は比較的高かった。したがって臨床現場への応用の可能性が示された。また先端医療における被験者への説明においてもこの質問紙は応用された。
