神経性大食症に対するR.Davisら(1990)の短期・期間限定の心理教育的グループ(Psychoeducation Group)の治療プログラムを,京都府立医科大学精神神経科外来のDSM-IVの診断基準を満たす女性患者13例に実施した。そのうち3例が治療中断したが,その理由は,転勤等の現実的理由や対人関係等の摂食障害以外の問題を有していたためであった。全プログラムを終了した10例において,過食や嘔吐・下剤乱用等の浄化行動は,2例で消失,5例で減少し,食行動異常や食事・体型への態度を評価するEAT(Eating Attitudes Test),EDI(Eating Disorder Inventory)の得点,不安・抑うつを評価するSDS(Self-rating Depression Scale)やSTAI(State-Trait Anxiety Inventory)の状態不安尺度の得点は有意に改善したが,強迫性を評価するLOI(Leyton Obsessional Inventory)の得点には改善は見られなかった。これらの傾向は半年後のフォロー・アップにおいても維持されていた。ビデオ録画等の検討からは,当初危惧された,体重に関するセット・ポイント理論の提示やグループでの治療への抵抗は大きな問題にはならなかった。以上より,本プログラムは我が国においても有用であると考えられた。その後,文章の平易化,社会的背景の部分の簡略化,グループワーク的な要素の省略等,プログラムを日本人に合う形に改変し,現在このプログラムを実施中であるが,現時点では治療効果の判定やこの治療が有用な患者の特徴を抽出できるほどには例数が集まっておらず,今後例数を増やし検討する予定である。
