絞扼性イレウスや虚血性腸炎、腸間膜動脈血栓症のような血行障害を伴う腸疾患の診断と治療法選択には緊急性を伴うが、特に年少者や高齢者では腹部所見に乏しく決定的な診断法もなく治療が遅れ救命不能な例もある。一方、動物実験では、腸管虚血ではその早期から腸管運動の変調がおこることが明らかになっているが、人でその変調を客観的定量的に評価する方法はまだない。そこで、これらの疾患を小腸電位の変調から早期診断することを目的に、犬を用いた上腸間膜動脈閉塞モデルを作製し、strain gauge force transducerにより実際の小腸運動を測定しながら、同時に体表からの小腸電位を測定し、病態の客観的評価が可能かどうか、またその早期発見に寄与できるかどうかという発想のもと以下を検討した。上腸間膜動脈完全閉塞による虚血後の小腸運動については、空腹期に胃から肛門側に伝播する強収縮波群が消失し、虚血後早期では食後期運動様の連続収縮波が観察された。また、小腸電位については、上部小腸由来と推定される10〜12cpmの成分のスペクトラム全体に占める割合が経時的に減少する傾向と伴に、虚血後早期から10cpmに満たない成分の相対的増加が認められた。以上から結果として、腸間膜動脈閉塞モデルでは、虚血により小腸運動が食後期運動様の連続収縮波に変化し次第に減弱するが、小腸電位では虚血後早期から収縮周波数の分布異常を来し、特に10cpmに満たない成分に注目することで病態の客観的評価ができる可能性が示唆された。

Diagnosis and treatment options for acute bowel disease, especially for the young, or for hypertension, including abdominal findings, are not available for treatment. The method for quantitative evaluation of intestinal movement in animals and humans is described in this paper. The early diagnosis of this disease is based on the measurement of intestinal motility by strain gauge force transducer, and the early detection of the disease is based on the evaluation of body surface intestinal potential. The upper intestinal artery is completely red, and the small intestine movement after blood deficiency is detected. The strong contraction wave group is disappeared in the stomach and anal side during fasting period, and the continuous contraction wave is detected in the early stage and late stage after blood deficiency. The potential of the small intestine was estimated to be from 10 to 12cpm, and the component of the upper small intestine was estimated to be from 10 to 12cpm. The component of the upper small intestine was estimated to be from 10 to 12 cpm. The above results show that the intestinal membrane is not normal, and the intestinal potential is abnormal in the distribution of contraction wave number in the early stage after blood deficiency, especially in the 10cpm period.
