新たな食道静脈瘤の薬物療法として,欧米において広く用いられているプロプラノロールに,門脈圧亢進の一因と考えられる血管収縮性ペプチドであるエンドセリンの拮抗薬(bozentan)を併用することで,より普遍的で効果的な門脈圧降下作用が期待される.ラット門脈圧亢進症モデルを用いて,エンドセリン拮抗薬併用の意義について検討し,さらに効果的な薬物療法の可能性について検討した.平成11年度の研究結果では,慢性肝疾患を伴わない門脈の段階的結紮による門亢症ラット(PVL群)では門脈圧降下作用に関してエンドセリン拮抗剤併用による相乗効果が得られないことが判明したため,平成12年度は胆管結紮による胆汁性肝硬変ラットを作成し併用効果を検討した.(方法)プロプラノロール,エンドセリン拮抗剤投与による全身および門脈循環動態の検討プロプラノロールは1mg/kgを7日間腹腔内投与,bozentanは10mg/kgを血行動態測定1時間前に静注し,門脈圧,体血圧ならびにradioactive microsphere法を用いた全身および門脈循環動態の測定を行った.(結果)胆汁性肝硬変ラットでは肝内門脈血管抵抗の上昇が認められ,血中エンドセリンの高値が観察された.プロプラノロールに加えbozentanを投与することによりに,体血圧,心拍出量の低下と共に門脈圧,門脈血流量のさらなる低下が認められた.(結語)肝硬変症などの慢性肝疾患を伴う門脈圧亢進症において,エンドセリン拮抗薬の併用投与により普遍的,効果的な門脈圧降下作用が得られることが判明した.今後は血圧低下などの副作用を最小限に抑制しうる投与量の決定が必要と考えられた.
