本研究は,耐性株高感度検出法を開発し,本邦におけるAcyclovir(ACV)導入以前の1970年代から現在までのHerpes simplex virus(HSV)臨床分離株を対象に薬剤耐性を量的・質的に検討し,さらに病変部でのウイルス量と耐性株の出現頻度からACV治療による耐性株出現の可能性を検討することを目的とした。その結果,本邦の1970年代から現在に至るまでの性器ヘルペス臨床分離株84株においては,年代によってもまたACVの急性期治療前後でも,ACV感受性,耐性株の出現頻度,phenotypeのパターンには変化が無いことがわかった。さらに1年から9年に渡る長期抑制療法によっても分離株のACV感受性,耐性株の出現頻度,Phenotypeのパターンは,未治療患者の分離株と変化が無いことが明らかとなった。耐性株高感度検出法を用いたことにより,耐性株は10^4PFUに数PFU存在することが明らかになり,再発病変中のウイルス量と耐性株の存在頻度という観点から見ると,ACV治療によっての耐性株発生の確率は非常に低いことが示唆された。このように,性器ヘルペス臨床分離株の耐性株の出現頻度を詳細に解析した本研究から,良く管理されたACV治療においては臨床上問題となるような耐性株の出現が無く,その可能性も非常に低いことが示唆された。
