我々は副腎腫瘍の有するmalignant potentialをテロメラーゼ活性の測定によって予測できるか検討するために本研究期間内に経験した副腎腫瘍手術検体を用いてテロメラーゼ活性を測定し、テロメラーゼ陽性症例について再発・転移の有無を追跡し臨床的有用性を明らかにしようと研究を行った。今年度に手術を受けた副腎腫瘍患者でinformed consentを得た上で、副腎腫瘍のテロメラーゼ活性を測定可能だったのは8症例、8検体だった。このうち1検体がテロメラーゼ陽性だった。陽性症例の病理診断は転移性副腎腫瘍(肺原発)だった。術後全身化学療法を施行後現在経過観察中である。テロメラーゼ陰性だった他の7検体の病理診断は良性副腎腫瘍だった。平成12年度とあわせると12検体のテロメラーゼ活性測定を行い、1検体(8%)で陽性だった。我々が本課題研究を開始する以前に1997年から行ってきた80検体の副腎腫瘍におけるテロメラーゼ活性の結果とあわせると、92検体中11検体(12%)でテロメラーゼ陽性だった。陽性だった11検体のうち病理学的に悪性と診断されたのは5検体だった。他のテロメラーゼ陽性6検体は病理学的に良性と診断された腫瘍だった。6検体の病理診断はそれぞれ、皮質腺腫2件、皮質過形成1件、褐色細胞腫1件、骨髄脂肪腫1件、副腎結核1件だった。これら6例のうち皮質腺腫と診断された1例で術後14ヶ月目に腹腔内再発を認め、褐色細胞腫と診断された1例で、術後36ヶ月目に局所再発、55ヶ月目に多発骨転移を認めた。両者とも悪性腫瘍としての臨床経過をたどった。以上の結果から、良性と悪性の鑑別が困難な副腎腫瘍におけるテロメラーゼ活性測定の有用性が示唆された。テロメラーゼ陽性腫瘍は病理学的に良性と診断された場合でも悪性の経過をたどり得る可能性があり、厳重な経過観察が必要と考えられた。
