1.正常妊娠における母体心拍数の日内変動を解明し、妊娠中毒症、塩酸リトドリン投与による影響を明らかにする目的で妊娠33-39週の妊婦19例((a)正常群11例(b)妊娠中毒症群4例(c)切迫早産による塩酸リトドリン投与群4例)に対してinformed consentを得た後、GMS社製長時間心電計AC301を用いて母体心拍数の24時間連続収録を施行。得られたデータより各群の母体心拍数日内変動の有無と群間での比較、日内変動振幅の変化を解析した結果、以下の知見を得た。(1)(a)(b)(c)全群に有意な日内変動を認めた。(2)(c)群は全区間で(a)群より有意に高く、(b)群は3-9時を除いて有意に低かったが、3群共に深夜で最も低く、15-18時に一過性の下降を示す同様の2峰性パターンを示した。(3)各群の平均日内変動振幅±標準偏差(bpm)は(a)19.1±10.0(b)13.0±5.0(c)15.0±6.4で3群間に有意差は認めなかった。以上の結果から母体心拍数には日内変動が認められ、妊娠中毒症、塩酸リトドリン投与による母体心拍数の変化は日内変動の位相、振幅には影響しないが、3-9時では母体心拍数日内変動の妊娠中毒症による修飾が示唆された。2.双胎妊娠における胎児基準心拍数の日内変動に対する母体心拍数、体動の影響と妊娠進行に伴う変化を解明するため、一絨毛膜二羊膜性双胎1例に対してinformed consentを得た後、妊娠31、34、36週に双胎心拍数、母体心拍数、母体体動の24時間同時収録を施行、各々の日内変動と相関関係を解析した。その結果、双胎間の基準心拍数日内変動位相は31週より既に同期しており母体心拍数、母体体動と高い相関を示すが、双胎間の同期にもかかわらず母子間で全く相関を認めない場合もあることが判明した。このことから胎児基準心拍数に対して母体心拍数、母体体動より更に影響の強い他の母体因子の存在することが示唆された。

1.
